Hello,
 Not a solution for the primary problem, but perhaps a reason for the
D6/xxxxx effect:

assuming you're not in \relative mode, try <<e g b d'>> so that d is not 
the base tone. Otherwise lilypond reads <<d e g b>>. 

But this should read D6/sus4/sus2  without 8 #8 or 10. ???
