bounding box

I seem to have a bounding box problem.  I made a web page with some
examples and one of the examples is missing the top staff line.  Upon
further investigation I found that the bounding box is cutting off the
top staff line and a bit of the bass clef.  Only one of the examples
suffers from this.  It can be seen here:

It is the fifth example down the page.  It looks fine if I just run it
through LilyPond but when I looked at one of the output files with gv I
saw that the bounding box didn't leave enough room at the top.  Now I
can't find that file again (???).

Anyway, I'm puzzled.  How do I fix this?
