I've got timidity working fine on OSX 10.3.  There's an English page
for timidity.  It wasn't hard to get working -- either there were
special instructions for OSX on the English page, or no special
instructions were needed (and you just got an old version).

After you've got timidity installed, you need a sound font or a
set of (musical, not programming) patches; IIRC I'm using the
EAWpatches.
