You may play around with \transparent, \combine and \fontsize get your
alignment right.

untested:
ppsempre =
    #(make-dynamic-script (markup #:combine #:transparent "f" "pp" #:combine
#:transparent "f" #:normal-text #:italic "sempre" ) )
