I can't remember fixing anything that could cause this. 

Can you check out what is happening to this beam? One option is to do

 \once \override Beam #'stencil = #(lambda (grob) (raise 1))

and run inside GDB.

I'd be interested in seeing the property lists of the beam, and each of 
the stems. IN particular, the 'beaming property of the stems.
