Thanks Mats.
Actually the bug is in the manual :( The ottava spanner is
created in the Staff context, but I omitted to specify the
context in the \override in the example.
\once \override Staff.OttavaBracket #'outside-staff-priority
= #340
works fine (at least in 2.11.34)
