the output from 

\version "2.11.38"

{<bes'! b'!>}

is what i want except for the flat needs to be moved to the left

i was hoping for something like:

\once \override Accidental #'extra-offset = #'(-2.0 . 0) -- but referencing the note within the chord

i can fake it by making an extra voice, hiding the stems, shifting the accidental, etc

(it's a pain, but the 'two-stave' or 'forked stem' or 'using ais instead of bes' solutions aren't appropriate here)

is there no way to do this with \tweak?

Could you provide a link to an example how that can be done (just take pencil
and paper, make a drawing, scan it and put it to the web)?
(I cannot imagine any solution than except for splitting the notes of the
"chord" (rather cluster?) into two staves or do you think a horizontal forked
stem?)
