Just add \once to your lines \set tupletSpannerDuration = #(ly:make-moment 5 48) and\set tupletSpannerDuration = #(ly:make-moment 3 28) You might want to increase the tuplet bracket padding, because the 7 crashes into the middle 3 in bar 2.
