One of the most gifted LilyPonders out there, Nicolas Sceaux, has
posted an example of title page at
http://lsr.dsi.unimi.it/LSR/Item?id=368
(beware, it requires the latest 2.11 series and will not work with
LilyPond 2.10.x)
I guess this also works to have a blank opening page, maybe using his
\vspace function.
