Hi Tom,

String numbers must also be placed inside chord constructs. See my warning here:

Also, normally in a three-voice texture like this, the bass voice is \voiceTwo and the middle is \voiceThree. \voiceTwo puts the stems, ties, fingerings, etc. down by default, whereas \voiceThree puts them up. You can always override at any point for special circumstances, of course.

Your \layout and \midi blocks were also outside the \score block and so were being ignored. I fiddled a bit and I think it's starting to clear up. (file attached) Don't forget when you're changing the single notes into chord constructs, to move the rhythmic value outside the chord construct. I often forget that. :)
