Inside the ledger lines, it can in fact not be seen clearly either.

I would be very interested to see any example of traditional music 
publishing that reproduces or approximates lilypond's usage. I'll note 
that the scan came from an IMSLP public domain score for Tschaikowsky's 
Romeo and Juliet fantasy [and you can find plenty of other scores with 
the same behavior]. As I've mentioned, I have played orchestral music 
for quite some time; violin parts often involve tremolo repeats on high 
notes, and I have never come across something of this kind - which would 
suggest that, at the very least, it is not common practice.
