A tremolo inside the staff? Very unusual in my experience. I would think it would be above the staff, where it would be clearly seen.
Inside the ledger lines, it can in fact not be seen clearly either.


I can say without a doubt that the lilypond output would be considered incorrect (and hard to read) by musicians.
I would say the same for the scan.
I would be very interested to see any example of traditional music publishing that reproduces or approximates lilypond's usage. I'll note that the scan came from an IMSLP public domain score for Tschaikowsky's Romeo and Juliet fantasy [and you can find plenty of other scores with the same behavior]. As I've mentioned, I have played orchestral music for quite some time; violin parts often involve tremolo repeats on high notes, and I have never come across something of this kind - which would suggest that, at the very least, it is not common practice.
