For a pitched trill on a single note, try terminating the trill spanner 
on a spacer note:

\relative c '' {

\pitchedTrill e2\startTrillSpan fis d\stopTrillSpan % as usual

\pitchedTrill e2\startTrillSpan fis s2\stopTrillSpan % with spacer note

}
