Indeed you are able to get this working on an external disk - which makes no real sense to me, only because if it works on an external disk it should work on an internal disk, the operating system is the same. The only difference is that the 'physical' path to the external disk from your desktop would be different than to the 'internal' disk.
The difference is that the external disc has 10.4.11 on it and the internal has 10.5.8. So it [v.2.12.2] works fine under 10.4.11, but not under 10.5.8. 

Q1. You have had a more fundamental problem from the screenshot you sent, and that is (please correct me if I am wrong) that you do not even get the 'file', 'Edit', 'Compile', 'Font', 'Window' or 'help' menus. At least that is what your very small screenshot seemed to show?
EXACTLY correct. Not only that but the program [v2.12.2] won't quit either. I have to "force quit" v2.12.2.   v 2.13.9 quits fine. 

Q2. Now when you run the Lilypond.app on your external disk you DO see these menus and you can compile the file?
Yes. works just fine from the System 10.4.11 external disc using v2.12.2. 

All done from Internal Hard Disc, System 10.5.8

Using LilyPond 2.12.2
I quit all applications and used Console. 
Here is the message I get in Console once LilyPond opens to the "Welcome to LilyPond" screen and has only the single "LilyPond" menu item in the graphic I sent. 

Dec 17 09:00:02 rdlpowerbook LilyPond[203]: +[NSATSGlyphGenerator initialize] invocation.  The class is deprecated.

As I said previously, I now have to 'force-quit' to get out of LilyPond. 

Using LilyPond 2.13.9
The program opens to the "Welcome to LilyPond" screen and the entire menu bar is present. 
I save the file to the desktop with an .ly file type. 
I hit "Compile->Text file" to try and compile the file in the window and get nothing at all. 
I opened the saved file from the desktop using the "open" command and tried "Compile->Text file" on it. Got nothing. 
The console shows this message:

I then used the 'update syntax' command from the 'compile' menu on the file that I had saved to the desktop and then opened and get this in the syntax window:

and this in the console window:
