Additional notes or ossia

Hello,

I have some scores that include instruction to change the levers at the 
celtic harp. This is shown in different styles, but best readable is an 
additional note with the sharp, flat or neutral sign. You find samples 
attached.

These additional notes should be surrounded with a rectangle. They are 
not played but the player adjusts his harp instead.

So let me describe, what I would like to write. (I know this sounds like 
a script for Santa Claus,but I hope that this makes it more clear.):

- they have no stem

- they are surrounded by a rectangle to indicate that a lever change is 
necessary

- the notehead is a bit smaller

- they occur at the start of a score but or at any place.

- they must be fit in the score as any other note, but dont affect the 
timing of real notes (see 2.png)

- The lever is adjusted with the left hand and there areless notes in 
the lower staff. Therefore you write sometimes a clef symbol before the 
additional note to indicate where the lever really has to be adjusted. 
But this must not affect the current clef in the lower staff.

An alternative would be a kind of ossia. But this needs additional space 
between the staves. That's why I would avoid it in most cases.

Do you have any hint, what should I try?
