Philip,

Here is an example of the first issue---sloping, and even in the wrong
direction---(it seems to occur only when a rest is involved):

\version "2.14.1"
\paper{ ragged-right=##t }
\relative c' {    
    \clef bass
    d4 d d( r  \break d')
}

The other issues all appear to be reported already.
