That did the trick.  Oddly enough, it still does not render correctly in Version 2.14.2, but when tried in 2.16.1 it renders perfectly.  I upgraded and problem solved.

Yes, you must specify the string tuning:

\set Staff.stringTunings = #mandolin-tuning
