Yes, it does occur. In the music we're typesetting (roiughly done...) for
the SCORA premiere, it occurs in a few spots in the Rachmaninoff, and I
believe one or two spots in the Sibelius-5. This is not obscure music. 

The code from the LSR does indeed not work nicely in all circumstances. A
better solution would be appreciated, but it's not very high on my priority
list.
