<http://lilypond.org/doc/v2.18/Documentation/notation/modifying-single-staves#ossia-staves> should help. For the labels, use two separate ossia staves and insert ‘instrumentName = "foo"’ into the \with {} block.
If there are further problems, it might help to post a compilable, small example.
