Certainly they can. The flags in their current shape are part of the music
font, so it’s not quite easy to just make them thicker, but you can create a
new ‘stencil’ (or someone on this list can) and replace the default one. The
other one is also feasible: do you mean the padding between individual
accidentals in a key signature?
In such case I would prepare the score in
Musescore and then import it into
Lilypond and do the rest.
Unfortunately, this is difficult. LilyPond has MusicXML import facilities, but
they aren’t quite satisfactory and tend to require a lot of manual adjustments.
There is ongoing work to improve this.
