Hello Donald,

I use PhotoScore Ultimate (not free, but quite worth its price) to scan PDF, 
fix the scanning issues, export to MusicXML and convert to LP with musicxml2ly.
The final fixes are done with Frescobaldi.

Can give great results with good quality PDF files.
