If I apply the attached patch to a cloned git archive I get the following results with ghostscript 9.06:

bok2: Original size 9.996.691 bytes, optimized size 2.043.380 bytes
bok3: Original size 13.706.324 bytes, optimized size 2.447.232 bytes

Unfortunately I found that a current git master of ghostscript does not work as intended. I have to bisect and investigate that problem.
Lovely! Thanks again!

I had a closer look at ghostscript.

Between ghostscript versions 9.16 and 9.17 commit 0ec0f1627b7f7f5ffa1347123a926cd1e32c9f19 was added.

That commit normally results in better handling of different subsets of the same font, but it breaks our existing solution.
Fortunately this commit also allows to define the PDFDontUseFontObjectNum option. If this option is used with ghostscript 9.17
or newer, the old behavior is restored.

Example for gs 9.17(+):
gs -dPDFDontUseFontObjectNum -dNOPAUSE -dBATCH -q -r600 -sDEVICE=pdfwrite -sOutputFile=bok3-opt.pdf bok3.pdf
