I tried Media5 with iOS 4.3.4 which is also free (plus another version with fee) and background with UDP works!!, using MiniSipserver on Windows. And works also without any keep alive setting enabled. But developers warn about the battery drain

It used to work from IOS IOS 4.0 to 4.2, but from my understanding, it has never been officially supported by Apple.
