For the record I think this is the link you are referring to for the upload script. http://lists.gnu.org/archive/html/linphone-developers/2013-08/msg00002.html here is a general discussion on how to write upload scripts for PHP http://www.w3schools.com/php/php_file_upload.asp it looks like the only output from the script should be the URL to the image which was uploaded. As in... so this should be a simple as setting up a server to receive the uploaded image and echo back its path and then setting the URL to the upload PHP script in Advanced settings.
