Log message:
        o Added new command \idx{} - To create an item with index entry
        \idn{} - To create an item
        both these commands are to be used with the description
        environment. Use of these commands will result in the
        description style of UTLK and should be used for all structures,
        parameters etc.
        
        Eg. The current style is:
        
        PAGE_OFFSET Start of the kernel address space blah blah...
        flag Flags which descrmmsds sddg fdgh dfkf
        
        The new style using these cmds will be:
        
        PAGE_OFFSET
        Start of the kernel address space blah blah...
        flag
        Flags which descrmmsds sddg fdgh dfkf
        
        which has much clear and well indented look.
        
        o Modified all existing descriptions to use the above cmds.
        o Added comments for couple of more function in chapter 5.
