Details:

Looking for manchester doesn't return manchester UK, but only 10 manchester
in the US. This is because nominatim limits to 10 results. Either ask the
nominatim folks to allow some "skip" param, or fix the code in maposmatic
(loop using exclude_place_id param to nominatim).
