Just to bring this pretty fully up-to-date, the IEEE-754-2008
standard, and much more are fully and clearly discussed in:
J-M Muller, et. al. Handbook of Floating Point Arithmetic,
Springer, 2010.

They even cover some points of SIMD operation and SSE up to SSE5.
