This works for me:

I cannot reproduce either... its a bit tricky on windows because you don't get proper errors from Python (because of the workaround for #43036).

But file a bug here if you if keeps happening: https://github.com/cbm755/octsympy/issues

BTW, "sympref" is the command you want in your last line.
