Yes. But it is the same user which uses different accounts.

Hard to say. We had only 2 occurances until now, and in both cases it 
surely was not restarted. We are trying to reproduce the problem by 
permanently logging in and out at the moment, but ... so far we did not 
find a way to reproduce it reliably.
