Turns out all the suspicions for AMD devices were correct, everywhere
we read a BAR address that the address matches the config space offset,
there's full access to PCI config space.  Attempt to generalize some
helpers to allow quirks to easily be added for mirrors and windows.
Also fill in complete config space for AMD.

This patch has been included in my vfio-vga-reset branch for some time,
but I guess I forgot to post it.  I'm doing that now with hopes of at
least including this much in a qemu-1.6 pull request.  For this version
I've made the 0x3c3 ATI quirk not conditionalized on the hardware read
value.  Several users have found this to be more reliable.  I've also
removed support for "old" ATI cards.  AFAIK, this means pre-RadeonHD.
These have never fully worked and there seems to be little point of
making them work.  A couple quirks and one variant of a quirk are unique
to these older cards, so we can remove a reasonable chunk of code by
dropping it.  And finally, updated to support the memory region owner
update.
