Internally, this means two nested 'beginDocument'
Consumer calls.

If you send me a self-contained and complete test-case
maybe I can look into it.

A testcase that runs stand-alone (without a servlet
environment) is preferable, but I can also debug something
that will run in Tomcat - if you give me complete
instructions and all the files.
