I checked http://cassius.its.unimelb.edu.au/~jtseng/Physiology.html

Currently there is only a notice of non-warranty.
You need to add a copyright notice (see
http://www.gnu.org/prep/maintain/html_node/Copyright-Notices.html for the
format) and a license notice (since the mBSD license is so short, you can
just copy the license itself, from
http://www.xfree86.org/3.3.6/COPYRIGHT2.html#5

I also installed OOo2 so I can check your documents in that format as well.
