Just some thoughts on this,

If your program is uploaded on GNU Savannah, then perhaps FSF's address would 
do? Savannah is the publisher (since they publish the program for public 
distribution), whereas you are the author (because you authored the program). 
The law appears to be about publishing.

FSF address, but maybe Savannah has a separate address?
