Already posted about this before, i had the exact same
problems you describe. Did a quicky script that checked
every few minutes and restarted the milter. Found out
a busy server with about 1500/day averaged maybe 3-4
restarts every 24 hours.
This was on a mostly stock redhat 7.3 with sendmail
from the updated redhat 8 src.rpm built on the 7.3
redhat box.
I was able to fix this completely by rebuilding the
sendmail package with IPv6 disabled. easy to do, i just
told it to build for redhat 6.2 but made sure milter
was enabled. I suspect if you rebuild your sendmail
without ipv6 support you may see similar improvements.
The milter now runs for weeks without any stalling.
I still run the check script just in case but have
yet to see it detect a stalled milter.
I now have four servers with this setup, one is busier
with about 5000+/day and the other two are small sites.
