report_safe and rewrite_header not working after FC3 upgrade

I have a really strange problem since upgrading to FC3.

Spam is still being tagged by Spamassassin as I'd expect and filed into a
junk folder as usual. However, the milter isn't performing actions on:

report_safe 1
rewrite_header Subject **SPAM(_SCORE_)**

In other word the messages are not being wrapped in a new message and the
subject isn't being changed.

If I change the required_hits line then after restart it picks that up so I
know SA and the milter are picking up parts of the local.cf file.

My version numbers are:

Kernel 2.6.10-1.770_FC3
sendmail-8.13.1-2.i386.rpm
sendmail-cf-8.13.1-2.i386.rpm
spamassassin-3.0.2-0.fc3.i386.rpm
spamassassin-tools-2.60-1.i386.rpm
spamass-milter-0.3.0-1.1.fc3.rf.i386.rpm

A sample from the logs shows the milter IS doing something:

But just not changing the subject or wrapping the message.

Anyone have any ideas?
