Out of interest I tried the conversion, under Linux ( not a Mac )
with SWFTools 0.9.0, using -v, the verbose flag. Did you?
All those GOTO links in the sizable index - from page 1000 onward -
cause the conversion to choke with an 'only supports 65536 shapes'
error. Plenty of references to this one littering the mailing list
archives. Not all suggested workarounds seem to help.

Up to page 1000, no problem. After that, try giving pdf2swf small
page chunks to chew on, i.e.
pdf2swf -v -p 1000.1080 -T 9 PDFReference15_v6.pdf -o
PDFReference15_v6_02.swf
then simply recombine the resulting swf's with swfcombine. Worked ok
that way when I tried it.
