* doc/texinfo.txi (setfilename): explain how file name is
determined
when there is no @setfilename.
(makeinfo options, HTML Translation): document new options and
update according to the changes in behaviour.
Separate --plaintext from --no-headers.
(Pointer Validation): @-commands in nodes are now supported.
