I have solved also my problems with the latex documentation; there is an
error in your file refman.tex, line 12. As stated in the fancyhdr manual,
pages 6ff, \footrulewidth must be set with \renewcommand and *not* with
\setlength; change that line to
\renewcommand{\footrulewidth}{0.4pt}
