Missing menu in cvs.texinfo

Hello!

The documentation for the current CVS from CVS cannot be processed:

$ makeinfo cvs.texinfo
./cvs.texinfo:9405: `Line group formats' has no Up field.
./cvs.texinfo:9576: `Line formats' has no Up field.
makeinfo: Removing output file `/usr/local/src/ccvs/doc/cvs.info' due to 
errors; use --force to preserve.

makeinfo is from texinfo-4.0b-3 (RedHat 7.2).

I guess that `Line group formats' and `Line formats' are meant to be 
subsections of `diff options'.  Then please apply the following patch:
