clarify global from_/to_server (aftermath)

The patches were tested by logging in to cvs.cvshome.org, checking out
trunk ccvs tree, and building it.  All went ok.


They should be applied after the "from-to-server-p" patch.  In fact, those
three are so closely related that it should have been called [1/3].  The
difference is that from-to-server-p patch is purely cosmetic, while two
others change actual code.


All of the three are against my previous "client" patches, but hopefully
could be applied against the stock tree (but it would require some
regeneration of "client" patches).


Next step (unless I'm stopped in mid-flight) is to extract out the
"kerberos-client".

After that server.c will undergo similar dissection.

Then authentication parts of client will move to a separate file.

Then authentication parts of server.

Then happiness.
