I have updated the doc/cvs.texinfo to move annotate around such that it
is now part of the generated doc/cvs.1 file.

I changed this patch slightly. I hope the revision helps clarify things.
If not, let me know.
For the other patches, we probably need more discussion and sanity.sh
test cases where possible to exercise them.
