Password-Location=SQL assumes that the actual password will be retrieved
from the SQL database, so it cannot be used with Password attribute.
The correct entry is:

DEFAULT Auth-Type = Local,
                Password-Location = SQL,
                Simultaneous-Use = 1
        Service-Type = Framed-User,
                Framed-Protocol = PPP
