[PATCH 04/34] man/terminfo.head: Reorganize for consistency.

...with other ncurses man pages.

1.  Define string for monospaced font name.
2.  Define strings to obtain typographer's quotes and certain ASCII
    characters.
3.  Define page-local macros.
4.  Define `n` and `d` strings for file formats' manual section number
    for file formats and parameterized terminfo database location,
    respectively.

Also use empty requests for visual separation in source, easing
maintainers' comprehension.
