Here's the patch. I left \thischapter{,num,name} untouched and added
new macros \pagechapter{,num,name} which are defined through the
marks. If this is ok, I'll try to document the new macros. If we
decide not to introduce the new macro names and reuse \thischapter*,
let me know, it's trivial to fix. Also, I didn't implement
\pagesection alternative for \thissection.
I also fixed a bug (I think). In \chapmacro, TeX inserts baseline
skip between \pdfdest and the actual heading, which is a potential
break point. So we need \nobreak there, otherwise \pdfdest (and now
also \mark) may end up on a page before the heading when
@setchapternewpage off.
