That's because gv.info was produced with makeinfo 4.8, which had a
bug: it would produce an Index entry even if the manual had no
indices. There's this snippet near the end of gv.info in the gv
distribution:

* Menu:
It shouldn't be there. When I produce the GV manual using the latest
makeinfo 6.4, this part is not generated.
IOW, you should report this bug to the GV maintainers: they should
refresh their Texinfo version.
