Summary: merge server and client book into a single one, in doc/ directory

* doc/demexp-book.tex: 

  - new merged internal documentation, that contains code of both the
    client and the server. This is more usefull as common code (net/ and
    lib/) is now shared;

  - fix credits and minor changes.

* Makefile: update it to produce the documentation.
