I can reproduce this the following way:
* delete my .denemo-2.0.21 directory
* launch denemo and decide not to take over my old settings
* View > Palettes > Select a palette to show > Gregorian Chant
* Click New on the palette
* Click Do1 on the palette
* insert notes with the PC keyboard, e.g. d e d c
* With each notes, a flexa is inserted as well.
