Bug can't be reproduced on last develop version or may be
obsolete because of changes, so we suppose it was silently fixed.
Because of this, we close the request.

Description:	icon's menu for opensurvey is not set.
Step to reproduce bug:	Activate module Opensurvey. Menu show up but no icon. It's shown in demo version but not in 3.5 branch or develop branch from git.
