* doc/misc/reftex.texi: Remove outdated instructions
(Building and Installing, Installation with make)
(Installation by Hand, Loading @RefTeX{}): Delete sections.
(Entering @RefTeX{} Mode): Merge into its parent.
(AUCTeX): Point to GNU ELPA for installation.
(Problems and Work-Arounds): Remove XEmacs-specific item.
