I was looking in the wrong direction.
#+BEGIN_SRC plantuml :file (make-temp-file "2018-03-18-testprefix-" nil ".svg")
(*) --> "step1"
--> "step2"
--> (*)
#+END_SRC
... works as expected and even defines the correct file extension.
