Both KDE and GNOME are known to have good accessibility support, and any
distribution that ships them should be usable by blind people. GNOME 3.6, which
is going to be released next week, will have a lot of accessibility
improvements so that you don't need to log out and log back in to turn on your
screenreader.
You can try it out from this LiveCD:
http://ftp.acc.umu.se/pub/GNOME/misc/testing/GNOME-3.5.92-LiveUSB.iso
... or from your favourite distribution when they start shipping it.
